We recognise that getting out and about isn’t easy for everybody. That’s why we offer a range of personalised support to assist you in continuing to do the things you love in your daily life. This might include:
Social Support
Community Care TASMANIA offers a range of social support from going to the movies, catching up with friend, and visiting family and loved ones.
Personal Support
Community Care TASMANIA offers assistance with attending doctor’s appointments and personal outings such as haircuts.
Ways to Fund Support in the Community
Community Care TASMANIA provides a range of options to help you fund support around the home. We understand that financial pressures can make it difficult to access the care you need, so we work hard to ensure everyone can receive services that suit their circumstances.
Our team can guide you through the application process, help you understand your eligibility, and clearly explain hourly rates and service fees. We offer two main ways to fund your support: through the Support at Home Program or by choosing self-funded care.
NDIS
The National Disability Insurance Scheme provides personalised funding for people with disabilities, and as a provider we deliver tailored support from personal care to community access and Allied Health to help participants build independence and achieve their goals with choice and control.
CHSP
The Commonwealth Home Support Programme (CHSP) provides entry-level assistance such as cleaning, meals, transport, and social support. It is designed to help older Australians remain safe, independent, and connected at home. Delivered through My Aged Care, it’s often the first step before higher-level services such as the Support at Home program.
The Tasmanian HACC Program
The Tasmanian HACC Program (Home and Community Care) supports older people and those with disabilities to live independently at home.
Veterans Home Care Program
As a Veterans Home Care provider, we deliver practical in-home support including domestic assistance, personal care, respite and home maintenance to help veterans and war widows live safely and independently at home.
Self-Funded Clients
We provide flexible fee for service support to Tasmanians who are self funded, waiting for funding, or ineligible for government programs, offering straightforward onboarding and personalised short or long term assistance.
