Support at Home

Support at Home is the Australian Government’s most comprehensive in-home aged care program. It is designed to help older Australians live independently at home for longer by simplifying the system and tailoring support to individual needs.

Eligible people receive a personalised budget through My Aged Care for services such as personal care, nursing, home maintenance, assistive technology and short term restorative care. The program also provides clearer pricing and more flexible care pathways, including end of life options.

Personal Care

Community Care TASMANIA can assist with showering, bathing, grooming, dressing, mobility assistance, continence support, and morning and evening routines.

Help Around the Home

Community Care TASMANIA can help with cleaning, laundry, dishes, changing linen, and general household tasks.

Meal Preparation and Nutrition

Community Care TASMANIA can help you with cooking the meals you enjoy, meal planning and shopping help, and with special dietary needs.

Community Nursing

We know how important it is to stay connected. Community Care TASMANIA, can help with home visits, getting to appointments, community outings, and support to stay connected.

Transport

Community Care TASMANIA can assist with transport to shops, appointments, and social activities.

Home Maintenance & Safety

Community Care TASMANIA can help with minor home repairs, safety checks, and gardening and outdoor help.

Allied Health

Community Care TASMANIA can bring specialists to you. We can assist with physiotherapy, occupational therapy, podiatry, and speech therapy.


Short-Term Care Programs

When you need a little extra support for a short time, our short-term programs are here to help. We can support you at home, assist with your health needs, or provide compassionate end-of-life care.

Restorative Care

Designed to help you regain strength and independence after illness or injury, this service offers a range of allied health and nursing support. We focus on building everyday living skills and helping you stay independent for longer. Support is available for up to 12 weeks, with the option of a further 4-week extension based on assessment.

End of Life Care

In-home end-of-life palliative care focuses on comfort, dignity, and quality of life. We provide emotional and practical support to help people feel relaxed and supported at home for as long as possible. Financial assistance is available to ease the burden during this time, with up to $25,000 in support and services available for up to 16 weeks.

Assistive Technology & Home Modifications (AT-HM) Scheme

This Australian Government scheme provides funding for assistive technology and home modifications to help you live comfortably and safely at home – without the need to save up first. These supports make everyday tasks easier and help you continue doing activities you may no longer be able to manage on your own.

The Tasmanian HACC Program

The Tasmanian HACC Program (Home and Community Care) supports older people and those with disabilities to live independently at home. It provides entry-level services like personal care, domestic assistance, transport, and social support for those awaiting or ineligible for government-funded programs, or for short-term support after a significant health event.

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Programme (CHSP) helps older Australians stay independent and safe at home and in their communities. It provides entry-level support such as cleaning, meals, transport, personal care, and social activities. Managed through My Aged Care, CHSP promotes wellbeing, independence, and social connection and often serves as a first step to higher-level aged care like the Support at Home program.

Department of Veterans Affairs

The Department of Veterans’ Affairs (DVA) In-Home Nursing program is an Australian Government-funded service available to eligible veterans, war widows, and their dependents. It provides access to qualified nurses who deliver clinical nursing care in the comfort of the person’s home, based on assessed health needs and a referral from a GP.

Services can include health assessments, wound and continence care, medication assistance, chronic disease management, and follow-up care after a hospital stay. The goal of in-home nursing is to support health, independence, and recovery while helping individuals continue to live safely and comfortably at home.

Self- Funded Clients

Community Care Champions offers fee-for-service support to clients across Tasmania. The service assists those unable or ineligible to access, awaiting access, or funded through other means- including insurance or self-funded arrangements. With a simple onboarding process and minimal paperwork, it’s available to all Tasmanians needing short- or long-term assistance.
