What you can expect with our personal care services
Wound Care
Our Registered and Enrolled Nurses provide comprehensive wound assessment, management, and monitoring in line with best-practice clinical guidelines and direction from your GP. This includes wound cleansing, dressing selection and application, infection prevention, pain management, and ongoing review. Early identification and intervention are prioritised to promote optimal healing and prevent complications.
Medication Management
Our clinical team supports safe and effective medication management through medication reviews, administration, prompting, and monitoring for side effects or changes in condition. We work closely with GPs and pharmacists to ensure medications are taken as prescribed and to reduce the risk of errors, missed doses, or adverse events.
Welfare Checks
Often, a general wellbeing check is required. Our clinical team will have a friendly discussion, monitor observations and take note of diet and fluid intake, sleeping and medication changes, and upcoming appointments.
Vital Sign Monitoring
As directed by your GP, our nursing team can monitor vital signs such as blood pressure, pulse, oxygen saturation, temperature, and blood glucose levels. Results are documented and reviewed to identify trends or early signs of deterioration, supporting proactive and preventative care.
Chronic Disease Support
For clients living with chronic conditions, our nurses provide ongoing clinical support tailored to individual needs. This may include symptom monitoring, medication support, health education, and coordination with your healthcare team. Our focus is on maintaining stability, promoting independence, reducing hospital admissions, and supporting you to live safely and comfortably at home.
Ways to Fund Community Nursing
Community Care TASMANIA provides a range of options to help you fund community nursing. We understand that financial pressures can make it difficult to access the care you need, so we work hard to ensure everyone can receive services that suit their circumstances.
Our team can guide you through the application process, help you understand your eligibility, and clearly explain hourly rates and service fees. We offer three key ways to fund your support: through the Support at Home Program, the Department of Veterans Affairs, or by choosing self-funded care.
Support at Home Program
Support at Home is the Australian Government’s most comprehensive aged care program helping older Australians live independently at home longer by simplifying the system and tailoring services to individual needs. Through My Aged Care, eligible people receive a budget for supports such as personal care, nursing, home maintenance, assistive technology, or short-term restorative care. The program offers clearer pricing, flexible care pathways, including end-of-life options, and ensures coordinated, high-quality support at home.
Department of Veterans Affairs
The Department of Veterans’ Affairs (DVA) In-Home Nursing program is an Australian Government-funded service available to eligible veterans, war widows, and their dependents. It provides access to qualified nurses who deliver clinical nursing care in the comfort of the person’s home, based on assessed health needs and a referral from a GP.
Services can include health assessments, wound and continence care, medication assistance, chronic disease management, and follow-up care after a hospital stay. The goal of in-home nursing is to support health, independence, and recovery while helping individuals continue to live safely and comfortably at home.
Self-Funded Clients
We provide flexible fee for service support to Tasmanians who are self funded, waiting for funding, or ineligible for government programs, offering straightforward onboarding and personalised short or long term assistance.
