Transport
Our team can work with you to ensure you’re transported safely for activities such as shopping, appointments or social activities. We recognise that getting out of the house is just as important as being cared for at home.
Home Maintenance and Safety
Keeping you or your loved one safe at home is an incredibly important aspect of care. We can work with you to coordinate and undertake minor home repairs, safety checks and gardening and outdoor help. These types of services aim to maintain your standard of living and keep you at home longer.
Allied Health
Allied Health Professionals play an important part in maintaining our wellbeing as we age. You may already be in touch with these services, but where you need further care or support we have a network of professionals including physiotherapists, occupational therapists, podiatrists and speech therapists to connect with.
Ways to Fund Our Services
Community Care TASMANIA provides a range of options to help you fund support around the home. We understand that financial pressures can make it difficult to access the care you need, so we work hard to ensure everyone can receive services that suit their circumstances.
Our team can guide you through the application process, help you understand your eligibility, and clearly explain hourly rates and service fees. We offer two main ways to fund your support: through the Support at Home Program or by choosing self-funded care.
Support at Home Program
Support at Home is the Australian Government’s most comprehensive aged care program helping older Australians live independently at home longer by simplifying the system and tailoring services to individual needs. Through My Aged Care, eligible people receive a budget for supports such as personal care, nursing, home maintenance, assistive technology, or short-term restorative care. The program offers clearer pricing, flexible care pathways, including end-of-life options, and ensures coordinated, high-quality support at home.
Self-Funded Clients
We provide flexible fee for service support to Tasmanians who are self funded, waiting for funding, or ineligible for government programs, offering straightforward onboarding and personalised short or long term assistance.
CHSP
The Commonwealth Home Support Programme (CHSP) helps older Australians stay independent and safe at home and in their communities. It provides entry-level support such as cleaning, meals, transport, personal care, and social activities. Managed through My Aged Care, CHSP promotes wellbeing, independence, and social connection and often serves as a first step to higher-level aged care like the Support at Home program.
The Tasmanian HACC Program
The Tasmanian HACC Program (Home and Community Care) supports older people and those with disabilities to live independently at home. It provides entry-level services like personal care, domestic assistance, transport, and social support for those awaiting or ineligible for government-funded programs, or for short-term support after a significant health event.
