Community Care TASMANIA can assist with referrals to My Aged Care to help make the referral process as seamless as possible. Health professionals can make a referral to My Aged Care to have their patient’s needs and eligibility for government-funded care assessed. Once assessed, individuals can get in touch with Community Care TASMANIA to begin their age-care journey.
